Westminster Baldwin Park in Orlando seeks a Director of Social Services / Admissions Coordinator to lead social services and admissions for a 20-bed rapid recovery unit. You will assess residents, coordinate admissions from hospital or home, educate families about Medicare/Medicaid, and maintain occupancy through outreach and relationship building.

Reporting to the Health Services Administrator, this full-time, on-site role requires a Bachelor’s degree in social work, sociology, or

• Bachelor’s degree in social work, sociology, health-related field or RN with at least two years experience.
• Three years’ experience in skilled nursing facility or medically related area is required.
• One year in long term care including Medicare, Medicaid and managed care protocols.

Responsabilidades

• Prepare comprehensive psychosocial assessments and care plans for all residents.
• Coordinate all admissions with transfers from hospital or home to facility.
• Tour facility with prospective residents/family members and assist with referrals as needed.
• Educate residents and families about Medicare/Medicaid coverage and co-insurance.
• Develop and maintain relations with physicians and hospitals.
• Manage discharge planning and ensure proper documentation.
