Job summary

Great Lakes Management is seeking a Resident Assistant in Plymouth, MN to provide hands-on care and companionship to residents, following each resident’s service agreement. The role supports dining, activities, and resident well-being under the supervision of a nurse and case manager.

Ideal candidates have at least six months in long-term care, CNA certification, and a patient, caring approach. Compensation ranges from $17.00 to $22.00 per hour, with shift differentials and 401(k) match benefits

Qualifications

• Six months of experience in long term care or assisted living residence (Preferred).
• Registered as a Nursing Assistant, Home Health Aide or Certified Nursing Assistant (Preferred).
• Ability to make responsible choices and decisions and act in the resident’s best interests.

Responsibilities

• Assist residents with ADLs as indicated in the service agreement.
• Help residents reach and maintain highest level of independence.
• Provide residents with physical and emotional support.
• Assist with treatments as delegated by the Registered Nurse per the service agreement.
• Observe residents and report changes in physical or emotional condition to the case manager.
• Interact with residents to provide reassurance and engagement.
• Complete daily documentation and care schedules.

Resident Assistant Job Objective(s):

The Resident Assistant is responsible for providing hands -on care, both physical and emotional, as outlined in each resident’s service agreement. R esponsible for effectively communicating with residents, families, and notifying the case manager of any changes in condition or concerns. The Resident Assistant assist s in the dining room and participating in activities and outings .

Resident Assistant Job Responsibilitie s :

• Help each resident to reach andmaintainthe highest level of performance and independence.
• Provide residents with both physical and emotional support.
• Assistwith treatments as delegated by the Registered Nurse per the resident service agreement
• The Resident Assistantobservesclientsand reportschanges in physical and emotional condition such as change in attitude, loss of appetite, weight, etc.tothe casemanager.
• Interactwith residents to provide reassurance, engagement, and redirection as appropriate.
• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.
