Job summary

Great Lakes Management seeks a dedicated Resident Assistant in Minnesota to provide hands-on care and support to residents in a long-term care setting. You will assist with daily activities, communicate with residents and families, and collaborate with the case manager to monitor changes in condition.

Qualifications

• Six months of experience in long term care or assisted living residence (Preferred)
• Registered as a Nursing Assistant, Home Health Aide or Certified Nursing Assistant (Preferred)
• Ability to make responsible choices and decisions and act in the resident’s best interests.

Responsibilities

• Assist residents with ADLs (Activities of Daily Living) as indicated on the individualized service agreement
• Help each resident to reach and maintain the highest level of performance and independence
• Provide residents with both physical and emotional support
• Assist with treatments as delegated by the Registered Nurse per the resident service agreement
• Observe clients and report changes in physical and emotional condition to the case manager
• Interact with residents to provide reassurance, engagement, and redirection as appropriate
• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.

Education

Registered as a Nursing Assistant, Home Health Aide or Certified Nursing Assistant
