Job summary

Great Lakes Management in West Saint Paul, MN is seeking a Resident Assistant to provide hands-on care to residents as outlined in each service agreement. You will communicate with residents and families and coordinate with the case manager for changes in condition.

This role supports dining, activities and outings, assisting with ADLs, and helping residents maintain independence. You will document daily care, follow the resident service agreement, and collaborate with a Registered Nurse on

Qualifications

• Six months of experience in long term care or assisted living preferred.
• Registered as CNA/HHA/CNAs preferred.
• Ability to make responsible choices in the resident’s best interests.

Responsibilities

• Assist residents with ADLs as indicated on the individualized service agreement.
• Help residents reach and maintain the highest level of performance and independence.
• Provide residents with both physical and emotional support.
• Assist with treatments as delegated by the Registered Nurse per the resident service agreement
• The Resident Assistant observes clients and reports changes in physical and emotional condition such as change in attitude, loss of appetite, weight, etc. to the casemanager.
• Interact with residents to provide reassurance, engagement, and redirection as appropriate.
• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.
