Job summary

Arcadia Care is seeking an MDS / Care Plan Coordinator to ensure timely, accurate resident assessments and interdisciplinary care plans in line with federal and state guidelines. You will identify acuity, determine care needs, and communicate expectations to families and responsible parties.

The role involves coordinating nursing, dietary, social/recreational, restorative, and physician services to support proper assessment and reimbursement, with responsibility for Medicare/Medicaid case-mix

Qualifications

• Must be Registered Nurse or Licensed Practical Nurse with current unencumbered state licensure.
• Long Term Care Experience preferred.
• Ability to read, write, speak, and understand the English language.

Responsibilities

• Integrates nursing, dietary, social recreation, restorative, rehabilitation, and physician services for accurate assessments and appropriate reimbursement.
• Completes Medicare/Medicaid case-mix documents to ensure reimbursement for care and services.
• Conducts ongoing MDS reviews to optimize RUG category.
• Oversees MDS/PPS documentation and submission processes.
