RowsOne in Milwaukee, WI is seeking a compassionate Social Worker for our skilled nursing and long-term care team. You will support residents' psychosocial needs, collaborate with nursing and rehab, and help ensure coordinated, person-centered care.

The role involves conducting assessments, developing care plans, and advocating for residents and families with a focus on dignity, rights, and safety within a regulated healthcare environment.

Formación

• Degree in Social Work or a related human services field; Bachelor's or Master's degree in Social Work preferred.
• Experience in skilled nursing, long-term care, rehabilitation, or senior care settings preferred.
• Strong communication, interpersonal, and problem-solving abilities.

Responsabilidades

• Assess residents' psychosocial needs and develop care plans and interventions.
• Provide ongoing support to residents and families throughout their stay.
• Participate in care plan meetings and collaborate with the interdisciplinary team.
• Assist with discharge planning and long-term care planning, and coordinate services as needed.
• Advocate for residents and families, ensuring clear communication with the care team.
• Complete social services assessments, documentation, and care plan updates in a timely manner.
• Ensure compliance with federal and state regulations related to social services.
• Build positive relationships with residents, families, staff, and external providers.
