Qualifications

• Bachelor's in Social Work required (BSW) or MSW.
• Active state social work license (CSW/APSW/ISW/LCSW) or equivalent.
• Minimum 1 year supervised social work experience in healthcare.
• Experience in LTC/SNF preferred.
• Ability to work independently and with interdisciplinary team.
• Proficient in documentation and EHR/MDS.

Responsibilities

• Assessment: identify residents' psychosocial, mental, and emotional needs through regular visitation and observation.
• Documentation: complete Social History and Psychosocial assessment of residents promptly upon admission and routinely thereafter, ensuring compliance with federal and state regulations.
• Care Planning: integrate assessment findings into the interdisciplinary care planning process (MDS).
• Advocacy: work closely with the interdisciplinary team to promote and protect residents' rights, dignity, and self-determination.
• Protection: prevent and address resident abuse, neglect, and exploitation as mandated by law and licensure.
• Discharge Planning: coordinate discharge planning services and link residents with post-discharge resources.
• Support: provide emotional support and counseling to residents and families coping with illness and long-term care placement.
• Other: duties as assigned by the Administrator.
