Job summary

Elderwood at Waverly in Waverly, NY seeks a Social Worker to join our skilled nursing and rehabilitation community. You will advocate for residents, address psychosocial needs, and coordinate admissions, care plans, transfers, and discharges to protect dignity throughout the care journey.

Work with a multidisciplinary team to support residents and families, ensure their voices are heard, and help with transitions while maintaining thorough documentation and adherence to regulations.

Qualifications

• Bachelor’s degree in Social Work or related field from an accredited institution.
• At least one year of supervised social work experience in healthcare.
• Strong communication, documentation, organization, and interdisciplinary collaboration skills.
• Genuine interest in working with older adults through healthcare transitions.
• Valid driver’s license and a driving record that meets company requirements.
• Proficiency in English to communicate with residents and families.

Responsibilities

• Manage an assigned social work caseload and serve as a consistent resource for residents and their families.
• Welcome new residents and support their emotional and social adjustment.
• Identify psychosocial, emotional, and practical needs and coordinate access to services.
• Participate in care plan development with the interdisciplinary team.
• Advocate for residents’ preferences, independence, dignity, and involvement in decisions.
• Support transitions, transfers, and discharge planning with the team.
• Complete admission and discharge assessments and document thoroughly.
• Provide counseling and connect residents with financial or community resources.
• Maintain accurate documentation and collaborate with nursing and medical staff.
