Responsibilities

• Complete comprehensive social service assessments and create individualized care plans.
• Actively participate in pre-admission and admission processes for proper placement.
• Provide ongoing emotional support and crisis intervention.
• Document psychosocial updates per facility policies and regulations.
• Facilitate individual and group support sessions.
• Develop and coordinate resident-focused volunteer programs.
• Orient new residents and families to the facility, services, and resources.
• Collaborate with the interdisciplinary team for holistic, resident-centered care.
