Job summary

Complete Care, a leading long-term care facility, is seeking a Social Worker to identify and address residents' psychosocial, mental, and emotional needs while protecting their rights and ensuring compassionate care.

You will collaborate with an interdisciplinary team to plan care, coordinate discharges, and support residents and families through illness and placement decisions. This role requires licensure and LTC experience in Wisconsin.

Qualifications

• Demonstrated ability to work independently while collaborating with the interdisciplinary team.
• Proficiency with computer-based documentation and EHR/MDS systems.
• Excellent ability to build rapport with residents, families, and peers.

Responsibilities

• Assess residents' psychosocial, mental, and emotional needs through regular visitation and observation.
• Complete Social History and Psychosocial assessments upon admission and during care.
• Support care planning by integrating assessment findings into the MDS process.
• Advocate for residents' rights, dignity, and self-determination.
• Prevent and address resident abuse, neglect, and exploitation as required by law.
• Coordinate discharge planning and link residents to post-discharge resources.
• Provide emotional support and counseling to residents and families.
• Perform other duties as assigned by the Administrator.
