The Medical Suites at Oak Creek in Oak Creek, WI is seeking a compassionate Social Worker to support residents and families through transitions in our skilled nursing and rehabilitation community. You will complete social histories and psychosocial assessments, develop individualized care plans with the interdisciplinary team, coordinate discharge planning, and advocate for resident rights and dignity.

Qualifications

• Wisconsin social work licensure in good standing.
• Bachelor’s degree in social work or related field.
• Experience in skilled nursing or rehabilitation settings preferred.
• Knowledge of discharge planning and care coordination.
• Familiarity with MDS, Medicare/Medicaid regulations preferred.
• Strong communication, organization, documentation, and problem-solving skills.
• Compassionate approach focused on resident dignity and quality of life.

Responsabilités

• Complete social histories and psychosocial assessments to identify residents’ strengths, preferences, support systems, and needs.
• Develop and update individualized social services care plans with the interdisciplinary team.
• Coordinate discharge planning beginning at admission, including referrals to community resources, home health services, and other appropriate supports.
• Provide emotional support and help residents and families adjust to admission, changes in health, and transitions in care.
• Participate in care conferences and communicate with residents, families, representatives, and healthcare partners.
• Advocate for resident rights, dignity, choice, and person-centered care.
• Assist with advance care planning resources, appropriate guardianship referrals, and access to financial or community assistance.
• Complete timely, accurate documentation and assigned Minimum Data Set (MDS) assessments.
• Help address resident concerns and connect individuals with appropriate behavioral health and other specialized services.
• Support staff education on the social and emotional needs of residents.
