Job summary

Complete Care seeks a Social Services Director in Chicago, IL to oversee psychosocial care, advocacy, and resident rights. You will lead assessments, care planning, and discharge coordination while partnering with an interdisciplinary team to ensure high-quality, person-centered care.

Qualifications

• Bachelor’s degree in Social Work (BSW) is required.
• Strong ability to work independently while collaborating with an interdisciplinary team.
• Proficiency in Electronic Health Record (EHR) systems and MDS documentation.
• Excellent interpersonal skills to build trust with residents, families, and staff.

Responsibilities

• Serve as the primary resident advocate and emotional support liaison.
• Conduct initial Social History and Psychosocial assessments on admission and at routine intervals.
• Integrate clinical findings into the Minimum Data Set (MDS) and care planning.
• Manage comprehensive discharge plans, coordinating community resources.
• Monitor for resident abuse, neglect, or exploitation per laws and licensing standards.
• Provide counseling and family support during long-term care transitions.
• Fulfill operational and clinical duties as assigned by the Facility Administrator.
