Job summary

Complete Care, Inc. in Chicago, IL is seeking a Social Services Director to advocate for residents' psychosocial and emotional needs while guiding the interdisciplinary care team.

The role emphasizes assessment, documentation, care planning, advocacy, protection, discharge planning, and family support, with a focus on upholding residents' rights and dignity in a long-term care setting.

Qualifications

• Bachelor's Degree in Human Science required.
• Experience in LTC or SNF strongly preferred.
• Ability to work independently and as part of an interdisciplinary team.
• Proficient in EHR/MDS documentation.
• Excellent rapport-building with residents, families, and peers.

Responsibilities

• Assess residents' psychosocial, mental, and emotional needs through regular visitation and observation.
• Complete Social History and Psychosocial assessments promptly upon admission and ongoing, ensuring compliance with regulations.
• Integrate assessment findings into the interdisciplinary care planning process (MDS).
• Advocate to promote and protect residents' rights, dignity, and self-determination.
• Coordinate discharge planning services and link residents with post-discharge resources.
• Provide emotional support to residents and families coping with illness and long-term care placement.
• Perform other duties as assigned by the Administrator.
