Career Strategies in Chicago north is seeking a Social Services Director for our skilled nursing facility in the Uptown Chicago–Edgewater area. This leadership role ensures residents receive emotional and psychosocial support and coordinates with residents, families, and the interdisciplinary team.

The ideal candidate has a Bachelor's in Social Work or Healthcare, prior long-term care experience, strong communication and discharge planning skills, and a passion for resident-centered care.

Qualifications

• Bachelor's degree in Social Work or Healthcare required.
• Strong knowledge of psychosocial assessment, care planning, and discharge planning.
• Excellent communication, organizational, and interpersonal skills.

Responsibilities

• Conduct comprehensive psychosocial assessments for residents.
• Develop, implement, and update individualized care plans.
• Assist residents and families with adjustment, discharge planning, and community resource referrals.
• Advocate for residents' rights and promote resident-centered care.
• Coordinate family meetings and provide support and counseling as needed.
• Ensure compliance with federal, state, and facility regulations regarding social services.
• Collaborate with the interdisciplinary team to address resident needs and improve outcomes.
• Maintain accurate and timely documentation.
