Qualifications

• Bachelor’s Degree in Social Work (BSW) is required.
• Active Illinois Social Work license (LSW/LCSW) in good standing is required.
• Experience in Skilled Nursing Facility (SNF) or Long-Term Care (LTC) settings preferred.

Responsibilities

• Serve as the primary resident advocate and emotional support liaison, upholding rights and dignity.
• Perform admission social histories and psychosocial assessments.
• Incorporate clinical findings into MDS and interdisciplinary care planning.
• Coordinate discharge plans with community resources and home care services.
• Monitor for and report resident abuse, neglect, or exploitation.
• Provide counseling and support to residents and families during transitions.
• Collaborate with Facility Administrator on operational and clinical duties.
