Job summary

Inglis seeks a Social Services Coordinator to secure and coordinate behavioral health and psychosocial support at Inglis House in Philadelphia. You will advocate for residents, develop assessments, and partner with families and staff to ensure person-centered care plans.

Responsibilities include documentation, attending care conferences, and supporting discharge planning and grievance processes. Bachelor's in social work or human services plus supervised experience preferred.

Qualifications

  • Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field.
  • One year of supervised social work experience in a health care setting working directly with individuals.
  • Ability to function independently with flexibility, integrity, and teamwork.
  • Ability to relate to and work with ill, disabled, elderly, emotionally upset and hostile people.
  • Ability to make independent decisions and develop psychosocial assessments and care plans.
  • Excellent listening and communication skills.

Responsibilities

  • Coordinate and secure behavioral health services and psychosocial support for residents.
  • Develop and implement care plans, document progress, and support family and resident needs.
  • Attend care conferences and ensure resident rights and preferences are considered.
  • Support grievance investigations and discharge planning as needed.
  • Complete mandatory documentation within regulatory timelines.
