Your Impact
The Social Services Coordinator has the primary responsibility of securing, supporting, and coordinating behavioral health services and psychosocial support that encourage and sustain an environment that is least restrictive to the psychosocial health of a resident of Inglis House. The Coordinator provides concrete services, evaluates mood, monitors behavior, and implements appropriate interventions, as evidenced by developing behavioral plans when needed, which may include working with resident families and their concerns. The Coordinator serves as an advocate for residents and a liaison to the facility, family members, and representatives from other entities. All essential job responsibilities are executed in accordance with Inglis Administrative procedures and compliance/ethics guidelines.

What You'll Do
Essential Functions
Concrete Services to Residents
- Ensure transportation applications are filled out for residents within the indicated deadline.
- Be assessable and responsive to residents’ and/or family’s requests and ensure phone calls are promptly returned within 24‑48 business hours.
- Coordinate and facilitate concern processes and refer concerns to appropriate staff for resolution.
- Support grievance investigations and communicate the resolution to grievances to residents and/or their representatives as needed within the timeframe specified by Inglis House policy. Assist with discharge planning as needed and requested by the Director of Social Services.
- Assist with additional tasks and responsibilities as deemed necessary by the Director of Social Services and Nursing Home Administration.
Advocating for Residents of Inglis House/Residents Rights
- Develop a comprehensive social history and psychosocial assessment that includes the resident’s areas of need, strengths, and preferences, including cultural, religious, ethnic, and mental health background and implications for the care plan.
- Provide a consistent forum for helping residents express their needs, concerns, and wishes.
- Involve residents, families, and designated representatives in care planning and encourage their attendance at care conferences.
- Attend scheduled care conferences and identify and interpret psychosocial needs of residents for inclusion in the care plan, addressing issues of schedule, treatment, and environment that express and reinforce individuality and identity.
- Complete mandatory documentation within timelines established by applicable regulations.
- Support residents during meetings and discussions to ensure resident’s needs and preferences are considered.
- Inform residents of Ombudsman services when needed.
- Schedule and facilitate family meetings as needed.
- Inform residents and families of services to assist with completion of Advanced Directives, Living Wills, Power of Attorney paperwork, etc. during admission process and when needed and support as needed.
- Report and address possible allegations of abuse and/or neglect as they occur.
- Interview affected residents and other residents as determined when allegations are made and assist with securing identified areas of support.
- Document findings of reported allegation and report to Nursing Home Administrator within 24 hours of findings.
- Review and document resident rights with resident on an annual basis.
Documentation
- Progress notes for behavioral concerns are completed and charted within one (1) business day of resident contact.
- MDS are completed within five (5) business days of the assessment reference date.
- Social work assessments are completed and charted within fourteen (14) days of admission.
- Complete quarterly care conference documentation within one business day.
- Review Living Wills and Advanced Directives for accuracy during care conferences and as requested.
- Complete quarterly and annual notes and review for accuracy within the specified time frame as determined by the supervisor.
- Update, revise, and adjust residents’ care plans based on MDS assessments, witnessed and reported behaviors, psychotropic medications, discharge disposition, and identified and discussed psychosocial and behavioral health supports. Care plans address person‑centered care approaches and interventions attempted to meet residents’ needs.
Behavioral & Mental Health Interventions
- Complete quarterly assessments and quarterly notes.
- Attend quarterly Gradual Dose Reduction Meetings.
- Consult with Director of Social Work, counselors, and psychiatrist to develop behavioral plans and contracts as needed.
- Consult with interdisciplinary team, Nursing Home Administrator, resident, and support systems to develop behavioral/conduct agreements as needed.
- Consult with residents, support systems, and interdisciplinary teams to develop and implement person‑centered interventions for residents of Inglis House.
What We're Looking For
Education and Work Experience
Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field (required) including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology.
One year of supervised social work experience in a health care setting working directly with individuals.
Must be able to function independently and have flexibility, personal integrity, and the ability to work effectively with residents, staff, and support agencies.
Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people within the facility.
Must possess the ability to make independent decisions when circumstances warrant such action. Ability to make psychosocial assessments and develop and implement viable care plans.
Excellent listening and communication skills.
Pay Rate for this Role
$55,227 - $72,220 annually
