Complete Care is hiring a Social Worker to identify and address the psychosocial, mental, and emotional needs of residents, ensuring their rights are upheld. You will advocate for residents and collaborate with an interdisciplinary team to deliver person-centered care.

The role emphasizes documentation, care planning (MDS), and discharge planning, with a focus on supporting residents and families through illness and long-term care placement.

Qualifications

• Bachelor's degree in Social Work required.
• Active WI social work license required.
• 1+ year in health care setting preferred, LTC/SNF preferred.

Responsibilities

• Assessment: identify residents' psychosocial, mental, and emotional needs through regular visitation and observation.
• Documentation: complete Social History and Psychosocial assessment upon admission and regularly thereafter.
• Care Planning: integrate assessment findings into the interdisciplinary care plan (MDS).
• Advocacy: promote resident rights and self-determination with the interdisciplinary team.
• Discharge Planning: coordinate smooth transitions and link to post-discharge resources.
