Atrium Centers is seeking a Social Services Assistant to support the development and execution of social service programs. You will help identify residents’ social, emotional, and psychological needs and coordinate service delivery with families and interdisciplinary teams.

Qualifications

• Associate’s degree preferred, experience considered in lieu of degree.
• Experience in skilled nursing or long-term care preferred.
• Strong written and verbal communication skills.
• Ability to assist with documentation, assessments, care planning, and regulatory requirements.
• Compassionate, resident-focused approach with strong interpersonal skills.
• Organized and detail-oriented with ability to manage multiple priorities.
• Knowledge of resident rights, discharge planning, and psychosocial support preferred.
• Positive, can-do attitude with excellent attendance.

Responsibilities

• Assist Social Services Director in implementing social service programs per state and federal guidelines.
• Support identification and documentation of residents’ social, emotional, and psychological needs.
• Provide service coordination and follow-up for residents and families.
• Participate in care planning and interdisciplinary team meetings.
• Maintain accurate, timely documentation to support compliance.
