Job summary

Covenant Living in Columbus, GA is seeking a Social Services Assistant to support residents across levels of care. You will participate in assessments, coordinate services, and advocate for residents, families, and during transfers.

You will maintain documentation, assist with care planning meetings, and help residents select physicians while ensuring privacy and regulatory compliance. This role requires good communication, organized records, and a compassionate approach to aging and care

Qualifications

• High School Diploma or equivalent required.
• Associates degree preferred.
• Certifications in related areas are highly desirable.
• Entry level experience acceptable.

Responsibilities

• Assist as the facility representative with prospective residents, families and discharge planners for persons in need of skilled nursing care.
• Assist with orientation of residents to the skilled nursing facility and services.
• Collect and accurately record admission data and address questions and family adjustments.
• Maintain current documentation in the resident’s health record for social services in the care plan.
• Participate in resident care planning meetings for ongoing communication and problem-solving.
• Maintain contact with hospital discharge planners to assure continuity of care.
• Assist residents and/or families in selecting new physicians as necessary.
• Communicate information to departments regarding admission or discharge.
• Ensure care plan documentation meets regulatory requirements.
