Job summary

NHS Management, L.L.C. is seeking a Social Services Assistant to support resident social needs and coordinate services in our Daytona-area facility.

You will act as a liaison, assist in planning and implementing social care plans, complete forms, and participate in discharge planning with residents and families. A compassionate, patient approach is essential to help residents regain independence.

Qualifications

• Willingness to help others and work with elderly and disabled residents.
• Must have patience, tact, enthusiasm and a positive attitude.
• Must be able to speak, read, write and understand English.
• Must be at least 18 years old.

Responsibilities

• Act as liaison and representative of residents’ social interests and assist in planning and organizing the Social Services Department.
• Maintain resident rights reviews and advance directives; complete forms and progress notes.
• Participate in community planning and discharge planning to meet residents’ needs.
• Coordinate with residents, families, staff and community agencies.
