Job summary

Eagle Rock Post Acute Care at Cedar Grove is seeking a compassionate Social Worker to support residents and families with emotional support, discharge planning, care coordination, and advocacy to ensure the highest quality of care and quality of life.

You will conduct resident assessments and develop care plans, provide counseling, collaborate with interdisciplinary teams, assist with admissions and transitions, and ensure compliance with state and facility regulations.

Qualifications

• Bachelor’s or Master’s degree in Social Work (BSW/MSW).
• Current state licensure (as required).
• Experience in a skilled nursing or healthcare setting preferred.
• Strong communication, organizational, and interpersonal skills.
• Compassionate and resident-centered approach.

Responsibilities

• Conduct resident assessments and develop individualized care plans.
• Provide counseling and emotional support to residents and families.
• Coordinate discharge planning and post-acute services.
• Collaborate with nursing, therapy, and interdisciplinary teams.
