Job summary

MB Healthcare is seeking a compassionate Social Worker to join the Eagle Rock Post Acute Care at Cedar Grove team. The role supports residents and families with emotional support, discharge planning, and advocacy to ensure quality of care and quality of life.

The position requires a degree in Social Work and licensure as applicable, with experience in healthcare settings preferred. You will collaborate with nursing, therapy, and other professionals to coordinate services and ensure regulatory

Qualifications

• Bachelor’s or Master’s degree in Social Work (BSW/MSW) required or preferred.
• Current state licensure as required by the state.
• Experience in a skilled nursing or healthcare setting is preferred.
