Job summary

Avamere Rehab of Newport is seeking a Social Services Coordinator to support residents' psychosocial needs and coordinate safe discharge planning. This role reports to the Social Services Director and works with an interdisciplinary team to ensure compassionate, compliant care.

Responsibilities include coordinating with families, community providers, and payors; advocating for residents; managing care conferences; and assisting with Medicaid applications and post-discharge placements.

Qualifications

• Bachelor's degree preferred in public health administration, social work, or related field.
• 1 year of experience in a skilled nursing facility or similar health care setting.
• Experience with electronic medical records (EMR) systems.

Responsibilities

• Identify residents' emotional, mental, and psychosocial needs and ensure up-to-date care plans.
• Coordinate discharge planning with the interdisciplinary team to ensure safe transitions.
• Communicate with residents, families, providers, and payors on a regular basis.
• Advocate for residents' rights and ensure actions are in their best interest.
• Coordinate with community support services to ensure post-discharge needs are met.
