Job summary

Sonoma Post Acute in Sonoma, CA is seeking an Assistant Activity Director to support the Activities Director in delivering a creative, resident-centered program that meets physical, mental, and psychosocial needs.

You will help plan events, coordinate calendars, arrange transportation, document progress, and communicate with staff, families, and regulatory bodies to ensure quality and compliance. A high school diploma and ideally one year in long-term care are preferred.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios and proportions.
• Ability to solve practical problems.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Responsibilities

• Plan and conduct individual, small and large group activities.
• Assist in communication between employees at all levels, residents, families, support personnel, government agencies, and the public.
• Participate in community planning related to resident and family interests.
• Develop monthly activity calendar and maintain attendance records.
• Assist in developing appropriate plans of action to correct quality deficiencies as requested.
• Participate in discharge planning and development of activity care plans.
• Arrange transportation for residents for outings, appointments, or discharge.
• Develop monthly activity schedules including outings, group activities, and in-room activities for bed-bound residents.
