Job summary

Qualifications

• Must have high school diploma or equivalent.
• Prefer at least one year of experience in a long-term care facility.
• Ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.

Responsibilities

• Assist in planning and conducting activities for individuals and groups.
• Support communication between staff, residents and families.
• Help arrange transportation for residents as needed.
• Develop and help maintain monthly activity calendars and attendance records.
• Assist in discharge planning and activity care plans.
• Support residents in self-initiated activities and provide necessary materials.
