Job summary

Trellis Centennial in seeks an Activity Assistant to support the Activity Director in planning and executing a creative activities program designed to meet residents' physical, mental, and psychosocial needs in line with applicable standards.

Responsibilities include planning activities, coordinating calendars, and assisting with transportation, discharge planning, and progress documentation while fostering communication among staff, residents, families, and external agencies.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.

Responsibilities

• Participate in planning and conducting of individual, small and large group activities.
• Assist in providing good communication between employees, residents, families, and the public.
• Assist in development of monthly activity calendar and maintaining attendance records.
• Assist in discharge planning and development of activity care plans.
• Arrange transportation for residents when necessary.
• Develop a monthly activity schedule for residents, including outings and in-room activities for bed-bound or isolated residents.
• Encourage residents to participate in self-initiated activities including hobbies, crafts, and reading.
