Qualifications

• High school diploma or equivalent required.
• Prefer 1 year of experience in a long term care facility.

Responsibilities

• Plan and conduct individual, small and large group activities.
• Assist communication among staff, residents, families, and public.
• Develop monthly activity calendars and track attendance.
• Support discharge planning and resident assessments.
• Arrange transportation for residents as needed.
• Encourage resident participation in self-initiated activities.
