Zusammenfassung

Arbor Post Acute is seeking an Activity Assistant to support the Activity Director in delivering a creative, interactive program for residents. You will help plan and conduct activities, communicate with staff, families and providers, and assist in monthly calendars and documentation.

You will participate in care planning, coordinate outings when needed, and help maintain a safe, welcoming activity area. High school diploma required; LTC experience preferred.

• Ability to plan, organize and conduct activities for individuals and groups.
• Ability to adapt activities to residents’ interests and abilities.
• Comfort with documenting activity participation and progress notes.
• Ability to read and follow policy and procedures.
• Understanding of basic mathematical concepts for scheduling and attendance.
• Ability to interpret a variety of written and schedule instructions.
• Strong teamwork and collaboration with the care team.

Aufgaben

• Participate in planning and conducting of individual, small and large group activities.
• Assist in providing good communication between employees at all levels, residents, families, and the public to meet residents' needs.
• Participate in community planning related to the facility and resident services.
• Assist in development of monthly activity calendar and maintaining attendance records.
• Assist in taking part in quality improvement actions as requested by the Activities Director.
• Participate in discharge planning and activity care planning for residents.
