Job summary

Concerto Renal Services is seeking a PRN Social Worker to perform psychosocial evaluations and provide counseling for dialysis patients in skilled nursing or rehab facilities. You will coordinate with the interdisciplinary team, develop individualized care plans, and document in EMR to meet CMS guidelines.

Role requires a Master’s in social work, state licensure (clinical not required), and an unencumbered driver’s license.

Qualifications

• Master’s degree in social work.
• Healthcare or dialysis related background preferred.
• Must hold a valid state license; clinical not required.
• Unencumbered driver’s license.

Responsibilities

• Conduct psychosocial assessments and care planning with CMS guideline alignment.
• Provide counseling and education on dialysis, transplant options, and advance directives.
• Lead monthly care plan meetings with the interdisciplinary team and coordinate with facility staff.
• Maintain EMR documentation of assessments, interventions, and follow-ups; ensure CMS compliance.
• Advocate for patients, coordinate outpatient dialysis placement and community resources.
