Job summary

Concerto Renal Services is seeking a PRN Social Worker to support dialysis patients in Skilled Nursing or rehab facilities in Peoria, IL. You will perform psychosocial assessments, provide counseling, and help with care planning to enhance quality of life.

You will collaborate with an interdisciplinary team, ensure CMS guidelines are followed, and maintain accurate EMR documentation while supporting outpatient dialysis placement and discharge planning as needed.

Responsibilities

• Conduct psychosocial assessments (initial, 90-day, annual, and as-needed) and develop care plans with the interdisciplinary team.
• Provide counseling on dialysis, transplant options, and advance directives; educate patients.
• Lead monthly care plan meetings with patients and clinical staff, coordinate with facility teams, EQRS specialists, and regional managers.
• Maintain accurate EMR documentation of assessments, interventions, and follow-ups; track productivity and outcomes; ensure CMS guidelines adherence.
• Address patient and family concerns, assist with outpatient dialysis placement and discharge planning, and connect patients with community resources.
