Job summary

Concerto Renal Services seeks a PRN Social Worker to provide psychosocial evaluations, counseling, and quality-of-life support to dialysis patients in skilled nursing or rehab facilities. You will assess patients, develop care plans with the interdisciplinary team, and document in EMR per CMS guidelines.

The role reports to the market Team Lead and to the Director of Social Services. A Master’s in Social Work and state licensure are preferred; travel between facilities is required, with

Qualifications

• MSW required; state license preferred.
• Healthcare/dialysis background a plus.
• Ability to travel between facilities.

Responsibilities

• Conduct psychosocial assessments and develop care plans with the interdisciplinary team; align with CMS guidelines.
• Provide counseling and education on dialysis options and advance directives.
• Lead monthly care plan meetings with staff and coordinate with facility teams.
• Maintain accurate EMR documentation and ensure CMS policy compliance.
• Advocate for patients and coordinate community resources and discharge planning.
