Job summary

Concerto Renal Services is seeking a PRN Social Worker to deliver psychosocial evaluations, counseling, and quality-of-life support to dialysis patients in Skilled Nursing or rehab facilities.

You will conduct assessments, develop care plans with the interdisciplinary team, and ensure timely CMS-guideline based documentation. This role requires strong communication and collaboration within a multidisciplinary setting.

Qualifications

• Master’s degree in social work.
• Healthcare or dialysis-related background preferred.
• Must hold a valid state license; clinical not required.
• Unencumbered driver’s license.
• Intermediate computer skills; proficient with MS Word, Excel, Outlook and Concerto applications within 60 days.

Responsibilities

• Conduct psychosocial assessments (initial, 90-day, annual, and as-needed).
• Provide counseling and education on dialysis, transplant options, and advance directives.
• Lead monthly care plan meetings with patients and clinical staff.
• Maintain EMR documentation of assessments, interventions and follow-ups, and ensure CMS compliance.
• Advocate for patients, coordinate community resources and discharge planning.
