Job summary

Concerto Renal Services in Massachusetts seeks a PRN Social Worker to provide psychosocial evaluations, counseling, and quality‑of‑life support to dialysis patients in skilled nursing or rehab facilities. You will conduct assessments, participate in interdisciplinary rounds, and help with care planning in alignment with CMS guidelines.

The role reports to the market Team Lead and the Director of Social Services; requires a Master’s in social work, a state license, and strong communication

Qualifications

• Master’s degree in social work.
• Healthcare or dialysis related background preferred.
• Must hold a valid state license; clinical not required.
• Unencumbered driver’s license.
• Intermediate computer skills and proficiency with MS Word, Excel, Outlook as well as becoming proficient with Concerto specific applications within 60 days of employment.

Responsibilities

• Patient Assessment & Care Planning: Conduct psychosocial assessments and develop care plans with the interdisciplinary team.
• Therapeutic Support & Education: Provide counseling on dialysis, transplant options, and advance directives; develop patient education materials.
• Interdisciplinary Collaboration: Lead monthly care plan meetings with patients and clinical staff and coordinate with facility teams.
• Documentation & Compliance: Maintain EMR documentation and ensure CMS guideline compliance.
• Advocacy & Resource Coordination: Address family concerns, assist with discharge planning, and connect patients to community resources.
