Job summary

Concerto Renal Services is seeking a PRN Social Worker to provide psychosocial evaluations, counseling, and quality-of-life support for dialysis patients in Skilled Nursing or rehab facilities in Greenville, MI.

Join a multidisciplinary team, ensure CMS-compliant care planning, and maintain detailed EMR documentation while coordinating with EQRS specialists and regional managers. Flexible schedule with community resources advocacy.

Qualifications

PSYCHOSOCIAL assessments and care planning in coordination with the interdisciplinary team.
Counseling and education on dialysis, transplant options, and advance directives; support for adherence and quality of life.
Documentation to CMS guidelines and QAPI standards.
Collaboration with facility teams, EQRS specialists, and regional managers; supports new openings and admissions.

Responsibilities

Patient Assessment & Care Planning: conducts psychosocial assessments and develops care plans with the team.
Therapeutic Support & Education: provides counseling on dialysis and advance directives; creates patient education materials.
Interdisciplinary Collaboration: leads care plan meetings and coordinates with facility staff and regional teams.
Documentation & Compliance: maintains EMR records and ensures CMS QAPI compliance.
Advocacy & Resource Coordination: connects patients with community resources and manages grievances.
