Job summary

Interwell Health, a kidney care management company, seeks Renal Care Coordinators (RCCs) to embed within local nephrology practices in Longview, TX. The RCCs coordinate care for late-stage CKD patients, offering support, education, and care management to help patients start dialysis smoothly. This full-time role includes a $5,000 sign-on bonus and is based onsite.

This is a full-time, on-site position with possible light travel to nearby satellite locations; interview will discuss exact details.

Qualifications

• Minimum of 2 years experience in clinical renal patient care.
• Experience in renal transplant, dialysis, or CKD patient care preferred.
• Understanding of diabetes and cardiovascular disease processes preferred.
• Strong organizational and communication skills.

Responsibilities

• Assesses patient knowledge of late-stage CKD and educates to enable informed decisions during transition to RRT.
• Acts as liaison with staff to provide information on dialysis modalities and conservative care options.
• Organizes CKD education and transition planning for late-stage CKD patients.
• Participates in interpretation of summary clinical data to improve care processes.
