Job summary

UnitedHealth Group is seeking Wellness Coordinators in a remote nationwide role. You will provide care coordination and administrative support to members with behavioral health and substance use needs, acting as a primary care navigator and engaging members to address gaps in care.

Responsibilities include coordinating referrals, documenting in multiple systems, and assisting the Behavioral Health Case Management department. A 40-hour work week, Mon-Fri, PST or CST timezone is expected.

Qualifications

• High School Diploma required.
• 2+ years in behavioral health and case management.
• 1+ years computer proficiency.
• 1+ years dual diagnosis experience with mental health and substance use.
• Able to handle sensitive issues confidentially.

Responsibilities

• Inbound and outbound member queue handling.
• Documentation in electronic systems.
• Coordinate information with clinical teams.
• Provide behavioral health benefit education.
• Screen for gaps in care with members.
• Provide referrals and connect to community resources.
