Job summary

Tampa Family Health Centers is seeking a Value-Based Case Manager to advance value-based care strategies remotely, with occasional in-office attendance in Florida. The role focuses on proactive outreach, care coordination, and performance improvement aligned with quality measures.

Qualified candidates will have RN licensure (ADN acceptable), with BSN preferred, and 1–2 years in care management or population health.

Qualifications

• 1–2 years of experience in care management, case management, or population health in a primary health care or managed care setting.
• Graduate of an accredited School of Nursing; Florida RN license required or eligible.

Responsibilities

• Drive value-based care initiatives to improve clinical outcomes and reduce cost.
• Coordinate care and outreach for high-risk patients.
• Abstract and document clinical data for value-based reporting.
• Prepare and present value-based data to leadership and carriers.
• Collaborate with care team to implement evidence-based protocols.
