Job summary

Cardiovascular Services of America (CVAUSA) is seeking a Registered Nurse Care Manager to support our value-based care program. The remote role requires Florida residency and focuses on clinical oversight and care coordination for cardiac patients under value-based agreements.

The RN Care Manager will lead a POD with Medical Assistants and Health Coaches, manage triage and transitions, and ensure NCQA alignment with quality measures and program goals.

Qualifications

• Active, unrestricted RN license.
• Active compact RN license required (Illinois license a plus).
• Minimum 3–5 years clinical nursing experience, preferably in cardiology, care management, or population health.
• Experience in value-based care, managed care, or risk-based contracts.

Responsibilities

• Provide clinical oversight to a POD of Medical Assistants and Health Coaches.
• Lead care planning, enrollments, and program completion.
• Manage triage, high-risk care management, and transitional care for cardiac patients.
• Ensure timely follow-up, medication reconciliation, and patient education.
