Responsibilities

• Review referrals received daily in TruCare and perform timely telephonic outreach to eligible members.
• Explain care management services and obtain member participation.
• Complete assessments, identify barriers, and develop individualized care plans.
• Coordinate services, provide disease-specific education, and promote wellness exams and immunizations.
• Maintain accurate and timely documentation and manage a caseload of 75+; make ~400 calls per month.
