Job summary
Oak Street Health is seeking a Manager, Care Program Strategy & Operations (Vaccine) to lead vaccine initiatives across our population health platform. You will partner with clinical operations, nursing, providers, and IT to design and scale programs that improve immunization rates and care outcomes.
The role requires 5+ years in healthcare operations or related fields, strong project management, and proficiency with Epic and data integration.
Qualifications
- Bachelor's degree required.
- 5+ years in healthcare operations, population health, consulting, or related field.
- Experience leading complex cross-functional projects from planning to execution.
- Experience with Epic and healthcare data systems preferred.
Responsibilities
- Support vaccine and preventive care program strategy and execution across Oak Street Health.
- Collaborate with clinical operations, nursing, providers, and IT teams to design scalable workflows.
- Lead cross-functional initiatives and pilot programs from concept to implementation.
- Monitor KPIs for vaccine administration, care gaps, and data quality.
- Communicate program performance and risks to leadership and stakeholders.
