Job summary

Oak Street Health is seeking a Lead Director of Population Health to define clinical program requirements and drive technology enablement for complex care coordination. You will bridge clinical leadership and systems, partnering with informatics, product, engineering, and operations to shape requirements and measure success.

You will lead multi-team launches, develop clinical workflows, and translate requirements into buildable specifications.

Qualifications

10-15 years in healthcare program management, population health, or care delivery operations, with ownership of complex programs.
Deep working fluency with EHR systems (Epic strongly preferred) and ability to translate clinical needs into feasible technical requirements.
Demonstrated ability to influence technical and build teams as a non-owning partner - moving priorities through credibility and shared wins.

Responsibilities

Define and own clinical requirements for Population Health initiatives from design through go-live and optimization.
Develop prioritized clinical requirements to inform Epic/non-Epic technology roadmaps with Informatics and Product.
Translate clinical workflows into actionable requirements for technical partners.
Lead the clinical-operations workstream for multi-team launches.
Monitor program performance and flag issues early with analytics teams.
