Job summary

Moms In Motion in Virginia seeks a Service Facilitator (SF) Case Manager to visit clients in their homes, support families with Medicaid Waiver programs, and help ensure safe, healthy, independent lives. You will travel locally and work from a laptop, with no degree required and a focus on heart and experience.

Daily local travel, compassionate communication, and reliable transportation are essential as you guide families through plans, authorizations, and care assessments while building lasting

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; focus on heart and experience.
• Proficient with email, docs, spreadsheets, portal navigation and video calls.
• Valid driver’s license and reliable vehicle (no client transport).
• Strong written and spoken communication.
• Able to pass a background check and provide 2 professional references.
• Bonus if you have Person-Centered Thinking/Planning training.

Responsibilities

• Hit the road locally to visit clients in their homes.
• Be the go-to guide for families navigating waiver programs.
• Write up plans of care and assessments that actually make a difference.
• Troubleshoot challenges like service authorizations and timesheets.
• Build lasting relationships with families based on respect and trust.
• Work remotely from your laptop/tablet.
