Qualifications

• Current state RN license for MN/WI/CO or eligibility.
• 5+ years in home health, assisted living or long-term care.
• Strong organizational skills with minimal supervision.
• Caring, patient and empathetic toward elderly residents.
• Excellent verbal and written communication.
• Experience with Eldermark and PointClickCare EHR.
• Remote/hybrid capability and willingness to work weekends.

Responsibilities

• Handle triage calls from AL and MC sites during off-hours and weekends.
• Document directions and information in the EHR for clinical teams.
• Prepare activity and incident reports for site analysis and training needs.
• Participate in investigations or surveys for after-hours calls.
• Document appropriately in the EHR.
• Promote teamwork and a positive work environment.
