Job summary

Cassia is seeking an experienced Minnesota Registered Nurse for an After-Hours RN role that is part-time and remote. You will triage calls, coordinate care with hospitals, families and providers, and document all interactions in the EMR.

The position requires Minnesota licensure and may include cross-state training in IA, ND, FL and CO as needed. Experience in Assisted Living or DHS coordination is preferred.

Qualifications

• Current Minnesota RN license in good standing.
• Willingness to travel to training and Cassia sites.
• Experience in Assisted Living or long-term care helpful.
• Director of Health Services experience preferred but not required.

Responsibilities

• Triage after-hours calls from clinical staff regarding urgent resident situations.
• Assess, prioritize, and provide clinical guidance to ensure appropriate care.
• Coordinate care by communicating with hospitals, providers, families, and site leadership.
• Serve as an extension of site DHS to support clinical decision-making and continuity of care.
• Document calls, assessments, interventions, and outcomes in the EMR.
• Monitor and respond to calls throughout the entire shift.
• Communicate clearly with staff, residents and families.
• Build trust through responsiveness and expertise.
