Job summary

Molina Healthcare is seeking a Telephonic Care Manager, LTSS (LVN) who must live in Texas. The role supports long-term services and supports and coordinates with a multidisciplinary team to ensure effective member care.

Strong MS Office skills and 2+ years in health care are required. The position is fully telephonic with in-state residency, Monday–Friday 8 AM–5 PM CST, and may involve 25–40% local travel for contract needs.

Qualifications

• 2+ years health care experience, LTSS exposure preferred.
• License/certification only if required by contract/board rules, active if applicable.
• Bachelor’s degree in a health care field may be required in some states.
• Valid driver's license and ability to travel as required.
• Strong knowledge of community resources and Microsoft Office suite.

Responsibilities

• Provide telephonic care management and coordination for LTSS members.
• Develop and implement care plans with member and care team.
• Assess medical necessity and authorize waiver services.
• Facilitate waiver enrollment/disenrollment processes, and monitor progress.
• Coordinate with interdisciplinary care team meetings and document interventions.
• Travel 25–40% as required by state/contractual needs.
