Job summary

Molina Healthcare is seeking six RN care managers licensed in TX to work telephonically with Medicaid members across the state. Three roles require 2+ years in Behavioral Health or Substance Abuse; the other three in ICU/CCU/ED experience. Most work is telephonic with occasional in-person visits.

Hours are Mon–Fri, 8–5 in your time zone; bilingual is a plus. Solid experience with Microsoft Office, Outlook, Excel, Teams and OneNote is required.

Qualifications

• At least 2 years experience in healthcare, preferably in care management or related settings.
• RN license active and unrestricted in state of practice.
• Valid driver’s license and reliable transportation for travel requirements.

Responsibilities

• Perform comprehensive member assessments per timelines and determine eligibility for care management.
• Develop and implement care coordination plans with members, caregivers, and providers.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plans, document interventions, and adjust goals as needed.
• Maintain ongoing member caseload with regular outreach.
• Promote integration of behavioral health, LTSS, and community resources.
• Facilitate ICT meetings and collaboration among care teams.
• Use motivational interviewing and Molina guides to educate and support changes.
• Identify barriers to care and provide resources to address concerns.
• May provide consultation to peers and assist with complex caseloads.
