Job summary

Molina Healthcare is seeking a TX-licensed RN Case Manager to support members in women’s health within a managed care framework. This telework role involves telephonic assessments and coordinating care until discharge, with 25-40% travel as required.

The ideal candidate has case management experience in an MCO and strong MS Office skills, with Monday–Friday 8 AM–5 PM CST hours from home.

Qualifications

• At least 2 years in health care with 1 year in care management/LTSS.
• RN license active and unrestricted in state of practice.
• Bachelor's degree may be required in some states.
• Valid driver’s license and reliable transportation for travel.
• Proficiency with Microsoft Office Suite.

Responsibilities

• Perform comprehensive member assessments within timelines, including home visits as needed.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care team.
• Monitor care plan effectiveness and document outcomes.
• Coordinate services across behavioral health and LTSS for quality care.
