Job summary

Molina Healthcare is seeking an experienced Case Manager in Texas for a fully telephonic role. The candidate should have at least 3 years of Case Management experience or relevant health care experience, with LVN/LBSW/LMSW preferred but not required if experience meets criteria.

Role involves coordinating LTSS, waiver services, and collaboration with the care team, with travel 25-40% depending on state requirements. Office suite proficiency required.

Qualifications

• At least 2 years of healthcare experience, LTSS exposure preferred.
• Candidate may hold LVN, LBSW or LMSW if state contract allows.

Responsibilities

• Complete comprehensive member assessments within regulatory timelines.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with the member and care team.
• Monitor care plans and adjust as needed.
• Promote integration of behavioral health with LTSS services.
• Assess medical necessity and authorize waiver services.
• Coordinate with ICT meetings for care approvals.
