Fuze Health is seeking a Registered Nurse to support telehealth care for Medicare members, ensuring Stars compliance and successful ED discharge. You will perform virtual assessments, document care, and educate patients while collaborating with the care team to optimize outcomes.

The role requires BSN, active RN license, 2+ years of nursing, and 1–2 years in telehealth. Schedule includes weekdays with one Saturday monthly and EST hours. Competitive benefits included.

Qualifications

• BSN degree required.
• Active, unrestricted RN license in the U.S. state/compact license.
• 2+ years RN experience; 1-2 years in remote/telehealth.
• Strong computer skills and EMR familiarity.
• Excellent written and verbal communication.

Responsibilities

• Provide telehealth patient care remotely.
• Conduct virtual assessments reviewing history and current symptoms.
• Document patient conditions accurately and thoroughly.
• Educate patients and families on conditions and adherence.
• Collaborate with providers to develop individualized care plans.
• Maintain compliance with regulations and privacy laws.
